Which tax forms does OnPay file?
Withholding: Form IT-942: Semi Weekly Filers Quarterly Reconciliation, Form IT-501: Monthly Withholding Return, Form IT-501: Quarterly Withholding Reconciliation, Form IT-941: Annual Reconciliation/W-2 transmittal, and your monthly and quarterly municipality withholding taxes and reports.
Unemployment: Ohio Quarterly Tax and Wage Report
What information does OnPay need?
- Ohio Withholding State Tax ID: 8-digit number - Example: 12-345678
- Ohio Unemployment ID: 10-digit number - Example: 1234567-89-0
- Ohio Unemployment Rate
- Municipality/local tax ID & Filing frequency (if applicable)
NOTE: The Withholding and Unemployment are both paid through the Ohio Business Gateway (OBG.) If you have already set up an online account for the OBG then please click here for instructions on how to add OnPay as your Service provider.

You will need to have already registered your business with the Ohio Department of Job and Family Services and Ohio Department of Taxation. This information is required. If OnPay does not receive one or more pieces of the information/steps listed above, we may be unable to file/pay one or more of your taxes.
Where do I enter the information in OnPay?
If you already have your tax information, or if you have obtained it using the steps above, please update the tax information in OnPay:
- Login to OnPay
- Select the Settings menu icon
- Select Payroll Tax
- Update your Withholding Tax ID, Withholding Tax Deposit Schedule, Unemployment Tax ID, Unemployment Rate (as a %), and any local tax IDs as required.
I am an existing Ohio employer, where do I find this information?
If you are an employer with a history of payroll in Ohio you have likely obtained the IDs and online accounts necessary for OnPay to process your tax filings. If you cannot locate these documents, please call the appropriate agency at the number listed below.

Withholding Tax Account Number: You can find this on the Employer's Payment Of Ohio Tax Withheld (Form IT-501).
Ohio Department of Job and Family Services: (614) 644-2239
Unemployment Account Number: Existing employers can find the Unemployment Account Number on the Determination of Employer's Liability (form JFS 66314) or Contribution Rate Determination (form JFS 66314).
Unemployment Tax Rate: This can be found as the "Total Contribution Rate" on the Tax Rate Notice. The SUI rate for new employers is 2.7%. New construction employers use 6.4%.
I am a new employer, do I need to register for Ohio Taxes?
- Do you have a business and/or employees residing in the State of Ohio?
- Have you processed payroll in OnPay and have received warnings stating we are missing tax information?
- Have you received an email from an OnPay Tax Agent requesting certain Ohio information?
If you answered yes to any of the questions above, you should continue with the registration and setup steps below.
Which Ohio agencies should I register for?
Ohio Department of Taxation: You will need to register with this department if you have employees that both live and work in Ohio. If your business is located outside of Ohio, but your employees work or live in Ohio you will need to register with this agency.
Ohio Department of Job and Family Services: You will need to register with this department if you have employees that work in Ohio. If your business is located out of the state, but some of your employee's primary work location is in Ohio, you will need to register with this agency.
OnPay uses both the home address for each employee, plus the work site assigned to each employee to decide which taxes should be calculated.
